The Glasgow Evening Times reports on the phenomenon of junior doctors taking pay cuts when they become consultants, due to the level of banding payments which have also been linked with NHS cutbacks. MSP Pauline McNeil argued that "People who are working legally and within their time are benefitting from other people's overtime. This is the wrong way to operate an arrangement like this." However registrar Jason Long responded that "If the divisions want to cut their compliance costs they should do what they were supposed to do 11 years ago and cut doctors' hours."

Australian comment on NHS reforms 
The Medical Journal of Australia has published an article comparing modernisation in the Australian and UK health services. "The good news about the NHS modernisation agenda is that it exists, and is a valiant attempt to move forwards," the authors conclude. "It is, however, imposed from above, full of rhetoric, short on substance and poorly evaluated. It does not deal with the fundamental problems of the NHS — decades of infrastructure neglect, low staff-to-population ratios, chronic overload, and a focus on process rather than outcome."

BMJ has a whole issue dedicated to Electronic communication and health care. They are also running a webchat on this issue at 11am on Thursday 20th May.

The BMJ also reports on junior doctors' attacks on training reforms, and there are several direct responses to this article.

NHS Confed update on EWTD 
The NHS Confederation has published an EWTD update, which includes news on Hospital at Night.

The update also mentions that the European Commission are consulting on the impact of EWTD on health services, and that changes will probably not take place before "mid-2005".

News update 
The BBC reports on the role of nurses in driving NHS reform. "This is not about being a mini-doctor", said Beverly Malone from the RCN. "This is about being a maxi-nurse."

HR Gateway reports on a acute shortage of doctors in the Armed Forces, as well as separate concerns over the exploitation of overseas nurses.

Summary of guidance on doctors' hours 
Hampshire WDC have given us access to their excellent guidance document on doctors' hours. It's in the "Downloadable documents" section - highly recommended! Many thanks to Steve Harris at Hampshire WDC and Owen Melbourne at NW London WDC.

Junior doctors vote "no confidence" in training reforms 
Junior doctors have unanimously carried a vote of no confidence in Modernising Medical Careers, reports The Scotsman. BMA JDC chairman Simon Eccles emphasised that the process was welcomed, but argued "The implementation process is a mess".

The BMJ carries a number of letters in response to "No time to train the surgeons" article.

John Reid plugs Hospital At Night 
John Reid has told the HR in the NHS conference that "The Hospital At Night is a striking example of how the Working Time Directive can create better working lives for junior doctors and better services for patients".
